Instructions for Pre-Registration
To ensure timeliness on the day of your appointment and to alleviate any "first-time-patient" concerns,we encourage you to register and provide us with your medical history in advance. (Returning patients who have previously completed this form, either online or in our office, need not re-register.)
The four-step registration process (five steps for women) must be completed, in full, before information can be submitted to our office. As you complete each step, just click (next) at the bottom of the page to advance to the next step. At any time, you may return to a previous step simply by clicking on the section title to the left of the page.
